Trans Abstract

The frequency of procedures in the emergency department has increased with changes in the medical environment and the demands of the times. Especially in children, sedation and analgesia are often inevitable due to the difficulty in seeking cooperation. Procedural sedation and analgesia is essential for successful completion of procedure, but the medical personnel who perform it must be prepared for complications caused by medications. Safe procedural sedation and analgesia requires well-trained medical personnel and well-prepared equipment, including appropriate patient assessments and choice of medications, faithful monitoring, and resuscitation. This review focuses on understanding of sedation processes, patient evaluation, medications, and monitoring.

서론

1. 소아응급실 내 진정 및 진통 요법(procedural sedation and analgesia, PSA)의 특수성

응급실은 제한된 시간, 인력, 장비, 공간을 이용하여 환자의 문제점을 파악하고 해결해야 하는 특수한 환경의 의료 공간이다. 응급실 환자의 약 25%를 차지하는 소아응급환자(소아환자)는 낯선 응급실에서 진단과 치료에 필요한 시술 및 검사에 협조하기 어려워, PSA가 필요한 경우가 많다. 그러나 응급실의 한정된 의료자원으로 인해 환자 평가 및 감시가 불충분하여, 합병증 발생 시 제대로 대처하지 못하기도 한다. 특히 소아환자는 약의 치료범위(therapeutic range)가 좁아 예정보다 깊은 진정으로 빠지기 쉽다. 이는 성인에 비하여 해부∙생리학적으로 취약한 호흡계를 가진 소아에서 심각한 합병증 발생을 유발할 수 있는 위험 중 하나이다.

2. 진정의 정의 및 깊이

진정이란, 환자가 처치 시 통증을 느끼지 않도록 의식 저하를 유도하나 스스로 호흡을 유지할 수 있는 정도를 의미한다. 수술보다 소요 시간이 짧고 통증이 적은 술기에 적용하므로, 긴 시간 동안 환자의 호흡을 억제할 정도로 깊게 재우는 마취와는 확연히 구분된다. 진정 깊이는 3단계(얕은, 중등도, 깊은 진정)로 구분하며, 진정이 깊어질수록 호흡 및 순환에 미치는 영향이 커진다. 진정 깊이는 환자 상태와 술기 관련 통증 정도, 소요 시간 등을 고려하여 계획한다. 진정의 각 깊이는 뚜렷이 구분하기 어려운 하나의 연속선상에 있어, 흔히 예정보다 깊거나 얕게 진정될 수 있음에 주의해야 한다.

본론

1. 진정 전 단계

1) 진정 전 환자 평가 및 준비

PSA의 첫 단계는 환자 평가이다. AMPLE 등의 도구를 이용하여 병력을 청취한다. 이 도구의 이름은 A (allergy), M (medication), P (past medical history), L (last meal), E (event)로 이루어진 두문자어로, 병력청취의 필수 요소를 누락하지 않도록 도와준다. 또한, 과거력을 확인해 미국마취과학회(American Society of Anesthesiologists) 분류를 확인한다(Table 1) [1]. 대개 1, 2단계는 저위험군, 3, 4단계는 고위험군으로 분류하며, 고위험군에서 기도폐쇄와 같은 중대한 합병증 발생 가능성이 높다는 점을 알고 있어야 한다. 약의 금기에 해당하는 병력과 선천얼굴기형과 같이 처치에 어려움을 주는 징후를 파악한다.

2) 금식

응급실 진정에서 금식 기준은 곤란한 문제이다. 여러 진정 및 진통 요법 지침이 미국마취과학회 수술 전 금식 기준[2]을 준용하는 실정이지만, 응급실에서 소아환자에게 이 기준을 엄격하게 적용하긴 어렵다. 최근 시행된 소아 환자 대상의 두 개의 대규모 다기관 연구(한 연구는 응급실 소아 환자를 대상으로 함)에 따르면, 두 연구 결과 모두 진정 전 금식 기준을 지킨 군과 그렇지 못한 군 사이에 주요 합병증 발생 빈도에 차이가 없었다[3,4]. 실제로, 미국응급의학회(American College of Emergency Physicians) PSA 지침은 응급 시술이 필요한 환자에서 금식 상태는 고려 대상이 아니라는 점을 강조한다[5]. 소아환자 진정에서 금식 기준에 대한 새로운 합의가 필요하다.

3) 인력

진정 시술자 외에 진정 자체를 관리 및 감시하는 인력이 한 명 이상 필요하며, 이는 PSA 지침에서 필수적인 기준이다. 한 명의 시술자가 진정까지 담당하면, 시술에 집중하느라 환자 감시에 소홀할 수 있기 때문이다. 응급실 인력이 부족한 한국 상황에서 이 기준을 준수하기는 쉽지 않으나, 진정의 가장 중요한 항목이 환자 감시 및 합병증 대처임을 이해하고 노력해야 한다. 진정 담당 인력은 진정제 및 진통제의 특성과 작용기전을 잘 이해하고, 환자 평가 및 감시에 능숙하며, 합병증 발생 시 적절히 대처할 수 있고, 나아가 전문소아소생술(특히, 기도 관리)에 능숙해야 한다.

4) 장비

진정 전 장비 확인은 필수적이다. 장비 확인 절차는 SOAP-ME, 즉 S (suction: 흡인 기구), O (oxygen: 산소 및 산소 투여 장비), A (airway: 기도 유지 장비), P (pharmacy: 약), M (monitoring: 감시 장비), E (Extra-equipment: 정맥내주사 및 제세동기)로 알아 두면 좋다. 응급실 외에 다른 장소(예: 컴퓨터단층촬영실)로 이송하여 진정을 시행할 수도 있으므로, 실제 진정이 시행되는 장소에 장비를 갖추는 것이 중요하다.

2. 진정 단계

1) 환자 감시

얕은 진정 및 중등도 진정은 맥박산소측정기로 산소포화도와 심장박동수를 확인하는 것으로 충분하다. 반면, 깊은 진정은 안전한 시술을 위해, 추가로 심전도, 혈압, 호기말이산화탄소분압을 감시해야 한다[6]. 저환기 발생 후 산소포화도 감소까지는 어느 정도 시간이 걸리는데, 호기말이산화탄소분압 감시는 이를 조기에 확인할 수 있다. 깊은 진정, 고위험군, 두 종류 이상의 약 반복 투여 시 유용하며, 실제 propofol을 이용한 진정 연구에서 저산소증 발생 사례를 유의하게 줄였다고 보고했다[7].

2) 합병증 대처

진정 과정에서 기도와 호흡 관련 합병증이 가장 많다. 주로 혀 근육 이완으로 인한 기도폐쇄와 중추성 호흡억제가 대부분이며, 일반적인 기도유지, 분비물 흡인, 산소요법으로 호전된다. 성문연축은 드물게 발생하지만, 초기에 대처하지 못하면 치명적인 결과를 초래할 수 있다. 특히 소아환자에서 많이 사용하는 ketamine은 성문연축 발생 빈도가 다른 약보다 높아(0.3%-0.5%) [8], 발생 시 응급 처치 방법을 숙지해야 한다. 성문연축 발생의 위험인자는 상기도감염, 다른 진정제 또는 항콜린제 병용, 후두 후벽을 자극하는 시술(예: 위내시경) 등이다. 성문연축 발생 시 턱 들어올리기와 Bag-Valve-Mask Ventilation을 이용한 양압환기로 대부분 해결할 수 있으며, 양쪽 성문연축 패임(laryngospasm notch)을 동시에 세게 누르는 방법이 유용할 수 있다[9]. 위 방법에 호전되지 않으면 빠른연속기관삽관(rapid-sequence intubation)이 필요하므로, 의심되면 반드시 도움을 요청하고 필요한 약을 준비한다. 합병증 발생은 예측이 어렵고 낮지만, 일정한 빈도(예를 들어 ketamine 사용 때 기도와 호흡기 관련 합병증 발생은 약 4%)로 발생한다. 따라서 합병증에 대한 감시와 신속정확한 대처가 필요하다.

3) 진정 후 환자 관리 및 귀가 기준

진정제를 사용한 시술이 끝난 후 진정 효과가 지속하는 동안 환자 감시를 계속해야 한다. 소아환자 진정 합병증 중 시술 후 발생하는 비율이 약 8% 정도로 알려져 있다[10]. 응급실에서 PSA를 경험한 환자의 귀가 기준으로는 modified Aldrete scoring system이 가장 많이 쓰이며 총 9점 이상이면 귀가할 수 있다(Table 2) [11]. 귀가 시, 주의 사항과 연락처를 적은 안내문을 준다(Appendix 1).

4) 대표적 진정제 및 진통제

a. Ketamine

진정 및 진통 효과를 모두 가지며 소아에서 사용 경험이 장기간 축적되어 응급실에서 가장 많이 사용하는 약 중 하나이다. Ketamine은 대뇌 피질 기능을 억제하면서 변연계 기능을 촉진하여, 해리 진정제(dissociative sedative)로 분류한다. 독특한 기전만큼 다양한 부작용이 나타나므로 금기 기준을 숙지해야 한다[12].

나이 3개월 미만(기도합병증 위험 증가, 뇌 발달에 영향)과 조현병(조절 여부 무관)은 절대적 금기이다. 상대적 금기로는 인두후벽을 자극할 수 있는 시술(구강 내 검진 제외), 기도 수술력, 기관지협착, 호흡기감염, 심혈관계 질환, 신경계 종양, 수두증, 녹내장, 급성안구손상, 포르피린증, 갑상선 질환이 알려져 있다.

최근 연구에서 두부외상 또는 뇌수술 환자에서 ketamine이 유의하게 두개내압을 올리지 않는다고 보고했다[13-15]. 이 결과는 ketamine의 주요 부작용으로 알려졌던 두개내압 상승에 대해 재고할 필요성을 제시한다. 실제 미국소아과학회(American Academy of Pediatrics)의 PSA 교과서에서도, 약간의 호흡 양상의 변화에도 두개내압이 영향을 받을 수 있는 상황(두부종양으로 인한 뇌압상승 등)의 경우를 제외하고는 ketamine 사용을 주저할 필요가 없다고 기술하였다[16].

Ketamine 자체는 심장수축력을 감소시키지만 교감신경을 자극하는 기전으로 인해, 전체적으로는 혈압과 심장박동수를 증가시킨다. 교감신경 자극 기전은 카테콜아민 재흡수 억제로 추정한다. 따라서 카테콜아민이 부족한 중환자에서는 카테콜아민 자극 효과보다 심장수축력 감소 효과가 더 클 수 있어 주의가 필요하다[8].

  • - 작용 발현 시간: 30초(정맥내), 5분(근육내)

  • - 최대 효과 시간: 2-3분

  • - 지속시간: 5-10분(정맥내), 20-30분(근육내), 얕은 수면 경향은 최대 2시간 지속 

  • - 용량: 1-2 mg/kg (정맥내), 4-5 mg/kg (근육내)

b. Midazolam

응급실에서 널리 사용하며 소아에서 사용 경험도 많다. 기억상실 효과가 있으나 진통 효과는 없으며, 기도폐쇄와 중추성 호흡억제가 발생할 수 있다[17]. 일부에서 탈억제에 의한 과반응 현상(hyperactivity)이 나타날 수 있는데, 이 경우 midazolam 또는 다른 진정제를 추가 투여하거나 flumazenil을 사용할 수 있다[18]. 말초정맥내카테터 확보 없이 비강내로 투여할 수 있어 사용이 증가하고 있다[19].

  • - 작용 발현 시간: 1-2분

  • - 최대 효과 시간: 2-3분

  • - 지속시간: 30분

  • - 용량: 0.05-0.1 mg/kg (정맥내), 0.3-0.5 mg/kg (비강내)

c. Fentanyl

아편유사제로, 작용 효과가 뛰어나고 소아에서 사용 경험도 풍부한 편으로 진정 작용을 함께 보이는 경우가 있다. 호흡억제 발생에 대비해야 하며, 특히 다른 진정제 병용 시 주의해야 한다[17]. 최근 투여방법이 비강내 및 흡입 투여 등으로 다양해지면서 재평가되고 있다[20,21].

  • - 용량: 1-2 µg/kg (정맥내), 1-2 µg/kg (비강내), 3 µg/kg (흡입)

  • - 작용 발현 시간: 30초

  • - 최대 효과 시간: 2-4분

  • - 지속시간: 20분

5) 한국 소아 PSA 지침 및 교육의 필요성

소아 PSA에 대한 한국 지침은 2012년 대한소아응급의학회의 전신인 소아응급연구회가 발표한 “소아를 위한 술기 및 진정: 한국형 지침”이 시초이다[22]. 이후 대한소아마취과학회에서 “소아 진정 가이드라인: 한국형 지침”을 발표했다[23]. 두 지침 모두 소아 PSA에 대해 체계적인 설명과 자료를 제공한다.

전술한 대로, PSA 과정 중 약 부작용 및 합병증은 일정 비율로 발생한다. 따라서 안전한 시술이 가장 중요하며, 예기치 못한 합병증 발생을 조기에 인지하고 대처할 수 있는 지식과 기술을 익혀야 한다. 이에 소아청소년과 및 응급의학과 전공의 수련목표에 소아 PSA 항목을 포함해야 한다. 교육은 이론 강의에 모의 환자 시뮬레이션을 병행하여 진행해야 한다.

결론

응급실에서 시행하는 검사 및 시술 빈도 증가와 함께, 소아응급실 내 PSA 시행이 불가피해지고 있다. 안전한 PSA를 위해서는 인력과 장비의 준비는 물론이고 환자 평가, 약 선정, 환자 감시, 진정 후 환자 관리 및 퇴원 결정까지의 진정 전 과정이 면밀하고 꼼꼼하게 진행되어야 한다. 이 중 가장 중요한 요소는 이 과정을 관리하고 위기 상황에 대처하는 진정 담당 인력에 대한 충분한 교육이다. 현재 소아 진정 관련 수가가 논의 중인 것은 안전한 PSA에 대한 의료계 안팎의 높은 요구를 반영한 결과라고 생각한다. 이 수가 적용은 그만큼 진정 담당 인력에게 책임을 부여하는 의미로 해석할 수 있어, 이를 위한 인력의 교육, 의료자원 배치, 질 관리에 관심을 기울여야 한다.
