Trans Abstract

Purpose

We aimed to survey current status of preparedness for neonatal resuscitation in delivery rooms in Korea.

Methods

We analyzed data regarding preparedness for the resuscitation in terms of personnel, equipment, and education using a questionnaire, which was developed by the Korean Association of Cardiopulmonary Resuscitation. From August 2020 through January 2021, the questionnaires were sent to hospitals affiliated with the Korean Association of Maternity Hospitals or the Korean Neonatal Network (KNN) operated by the Korean Society of Neonatology. As per the affiliated society, the hospitals were grouped as the maternity hospitals or the KNN hospitals. The questionnaires were delivered in 3 steps: mailout, electronic mail, and phone calls.

Results

Response rate was 21.3% (115 of 541) and 65.3% (49 of 75) in the maternity and KNN hospitals, respectively. The maternity hospitals showed a higher mean number of delivery and fewer pediatricians. In the KNN hospitals, pediatricians played a leading role during the resuscitation, but in the maternity hospitals, their role as a leader was less conspicuous. The KNN hospitals were better equipped with masks of variable sizes, oxygen blender, T-piece resuscitator, and electrocardiographic monitor (all Ps < 0.001). In the maternity hospitals, the preparedness of oxygen blender or T-piece resuscitator was positively associated with the number of pediatricians (P = 0.005). Although bag-mask ventilation was mostly feasible in both groups, endotracheal intubation was less feasible in the maternity hospitals (P = 0.007). The annual participation in resuscitation training for providers was higher in the KNN hospitals (P < 0.001). In the maternity hospitals, the annual participation was positively associated with the numbers of deliveries, obstetricians, and pediatricians.

Conclusion

This preliminary survey indicates the need for further improvement in preparing in personnel, equipment, and education, particularly in maternity hospitals.

서론

신생아 소생술은 분만 후 심폐부전(cardiorespiratory compromise)에 처한 신생아가 소생하도록 돕는 의료행위로, 신생아를 관리하는 의료인이 관심을 가져야 한다. 출생 직후 자궁 내에서 자궁 외 환경으로 이행하는 과정에 적응하지 못한 신생아는 분만실에서 소생술을 받아야 한다. 전체 분만 중 약 10%는 신생아의 첫 호흡 개시 또는 호흡 확립을 위한 소생술, 1% 미만은 가슴압박 또는 약물요법을 포함한 소생술이 각각 필요하다1).

미국에서는 소아과학회(American Academy of Pediatrics, AAP) 및 심장협회(American Heart Association, AHA) 지침과 심폐소생술 국제소생술교류위원회의 국제협약이 5년마다 제작되고1-3), 일본은 자체적으로 소생술 표준지침을 만들고 보급함으로써, 신생아의 생명을 구하기 위해 노력하고 있다4). 한국에서도 2000년대 초반부터 대한신생아학회가 소생술 워크숍을 매년 개최하고 2017년 소생술 교과서 번역본5) 및 요약본6)을 발간했지만, 인식 및 보급이 아직 미흡하다. 대한심폐소생협회는 의료인 또는 기관마다 다르게 시행하는 소생술의 표준지침을 수립하고 소생술을 시행하는 모든 의료인 대상의 교재를 제작하기 위한 기초 자료로 활용하고자, 실태 조사를 시행하기로 했다. 이에 본 저자는 소생술 관련 인력, 장비, 교육에 관한 실태 조사를 통해, 한국 내 현황을 파악하고 이에 맞는 지침을 마련하는 데 활용하고자 본 연구를 수행했다.

대상과 방법

대한분만병원협회 및 대한신생아학회의 회원 명단에 기재된 병원 주소지로 사전에 준비한 설문지를 우편 발송했고(Appendices 1, 2; https://doi.org/10.22470/pemj.2022.00556), 응답하지 않은 기관은 2차로 동일한 설문지를 구글 설문지 형식으로 바꾸어 전자우편으로 발송했다. 응답하지 않은 기관을 대상으로(3차), 훈련된 조사원이 상기 설문지의 내용과 순서에 따라 전화로 질문했다. 설문지는 대한심폐소생협회의 도움을 받아 새로 개발했고, 2020년 8월-2021년 1월에 양 학회 회원 대상으로 각 병원의 2019년도 분만 건수, 산모-신생아 진료에 참여하는 소아청소년과 및 산과 의사 수, 고위험 분만 시 2인 이상의 의료인이 소생술에 참여하는지 여부, 소생술을 주도하는 의료인의 진료과목 또는 직종(소아청소년과, 산과, 마취통증의학과 의사 및 간호사), 복사온열기(radiant warmer), 호흡주머니(resuscitation bag), 다양한 크기의 산소마스크, 이송용 산소탱크(transporting oxygen cylinder), 맥박산소측정기, 후두내시경, 산소 혼합기(oxygen blender [흡입산소분율을 조절하는 장비]), T형 소생기(T-piece resuscitator), 심전도 감시장치, 후두마스크기도유지기를 포함하는 장비 현황, 소생술 중 시행할 수 있는 조치(백-마스크 환기[bag-mask ventilation], 기관내삽관, 가슴압박 및 약물요법), 연 1회 이상 소생술 교육 이수율 및 정기적으로 교육에 참여하는 직종별 비율을 조사했다.

설문 연구이므로 대상 집단의 특성을 파악해야 했는데, 제107차 대한산부인과학회 학술대회에서 보고된 바에 따르면, 한국의 분만병원은 2003년 1,373개에서 2019년 541개로 감소했다7). 대한신생아학회 회원을 대상으로 한 조사는 이 학회가 구축한 전국 단위의 한국신생아 네트워크(Korean Neonatal Network, KNN)에 참여하는 2, 3차 의료기관(KNN 병원)을 대상으로 했다. 본 연구는 가톨릭대학교 의과대학 성빈센트병원 연구윤리위원회의 승인(IRB no. VC22ZCSI0071)을 받아, 연구대상자 동의를 면제받고 진행했다.

결과

설문에 응한 병원은 총 164개소였고, 응답률은 분만병원 및 KNN 병원에서 각각 21.3% (541개소 중 115개소) 및 65.3% (75개소 중 49개소)였다. 응답한 병원은 두 군 중 한쪽에만 속했다. 응답한 병원의 지역 분포는 Fig. 1과 같다.

1. 연구대상 병원의 전반적 특성

164개소의 2019년 평균 분만은 795건, 고위험 분만 시 소생술에만 전념할 의료인을 2명 이상 확보한 곳은 96.3%였다. 소생술을 주도하는 의료인으로 소아청소년과 의사가 가장 많았고(67.7%), 복사온열기, 호흡주머니, 이송용 산소탱크, 맥박산소측정기, 후두내시경은 90% 이상에서 구비했지만, 양압환기를 위한 크기별 산소마스크, 산소 혼합기, T형 소생기, 후두마스크기도유지기 구비율은 모두 50%대였다. 소생술 중 시행할 수 있는 조치로는 거의 모든 병원에서 백-마스크 환기를 시행할 수 있다고 답했고, 기관내삽관 및 가슴압박이 가능한 비율은 그보다 낮았다. 연 1회 이상 소생술 교육 이수율 및 정기적으로 교육에 참여하는 비율은 각각 60% 및 70% 미만이었다(Table 1).

2. 분만병원 및 KNN 병원 비교

1) 인력

연평균 분만 건수는 분만병원에서 더 많았다. 산모-신생아 진료에 관여하는 산과 의사 수의 평균값은 차이가 없었고, 소아청소년과 의사는 KNN 병원에서 더 많았다. 소생술을 주도하는 의료인의 진료과목 또는 직종으로, 분만병원에서 소아청소년과 의사가 가장 흔했고(54.8%), 산과 및 마취과 의사, 간호사로 다양했지만, KNN 병원에서는 소아청소년과 의사가 거의 모든 소생술을 담당했다(98.0%).

2) 장비

KNN 병원은 다양한 크기의 산소 마스크, 산소 혼합기, T형 소생기, 심전도 감시 장치를 더 높은 빈도로 구비했다. 특히, 산소 혼합기는 분만병원의 43.5%만 구비했고, 분만병원보다 고위험 신생아 분만율이 높을 것으로 추정되는 KNN 병원도 81.6%였다. 분만병원 중 산소 혼합기 또는 T형 소생기를 구비한 곳에서 소아청소년과 의사 수의 평균값이 더 많았다(Table 2). 분만병원의 98.3%가 백-마스크 환기를 시행할 수 있다고 답했으나, 기관내삽관이 가능하다고 응답한 비율은 KNN 병원보다 낮았다.

3) 교육

KNN 병원에서 연 1회 이상 교육 이수율 및 정기적으로 교육에 참여하는 직종별 비율이 더 높았다. 분만병원 중 연 1회 이상 교육을 이수하는 곳에서 연간 분만 건수, 산과 및 소아청소년과 의사 수의 평균값이 모두 더 많았다(Table 3).

고찰

한국 및 미국 소생술 지침에 따르면 모든 분만에 신생아 초기 처치(보온, 머리 및 목 자세 잡기, 기도내 분비물 제거, 피부 닦아주기, 자극하기)와 양압환기에 전념할 의료진이 최소 1명1,8), 고위험 분만을 앞뒀다면 최소 2명이 각각 필요하며, 더 높은 수준의 소생술이 필요한 상황이라면 기관내삽관, 가슴압박, 약물요법을 시행할 수 있는 숙련된 소생술 팀이 참여하도록 권고된다6). 본 연구에서 분만병원의 96.3%에서 2인 이상의 의료인이 고위험 분만 시 소생술에 참여한다고 답하여, 인력 면에서 KNN 병원과 유의한 차이가 없었다.

분만실 내 소생술은 초기 처치, 양압환기, 기관내관 또는 후두마스크기도유지기, 가슴압박, 약물요법까지 알고리듬에 따라 시행한다1,6). 분만 후 신생아 중 10%는 초기 처치, 5%는 양압환기를 통해서 각각 자발 호흡을 개시하거나 확립할 수 있고, 0.4%-2%는 분만실에서 기관내삽관이 필요하다9). 본 연구에서 분만병원의 98.3%는 소생술 알고리듬에서 초기 처치 및 양압환기가, 82.6%는 기관내삽관이, 85.2%가 가슴압박 또는 약물치료까지 가능하다고 답했다.

장비 현황을 보면, 대부분의 분만병원에서 초기 처치에 필요한 장비를 구비했지만, 양압환기 장비는 미흡했다. 양압환기는 분만실 내 소생술에서 가장 중요한 단계로, 산소마스크와 호흡주머니를 이용한다. 이때 신생아 얼굴 크기에 맞게 마스크를 골라서 사용해야 하는데, 38.3%의 분만병원만이 만삭아 및 미숙아를 포함하는 다양한 크기의 마스크를 보유하고 있었다. 1993-2016년에 출생아 수는 43% 줄었지만 저체중아 또는 미숙아 비율은 2배 이상 증가한 것을 고려하면, 다양한 크기의 마스크가 필요하다10). 양압환기를 시작할 때 흡입산소분율은 임신나이 35주 이상의 신생아는 21% (실내 공기[room air]), 35주 미만은 21%-30%로 각각 투여하고, 이후 흡입산소분율은 건강한 만삭아가 출생 첫 10분간 실내 공기로 호흡했을 때 측정된 동맥관 전(pre-ductal) 산소포화도 목표에 따라 조절해야 한다고 권고된다6,11). 이를 위해, 산소유량계 이외에 압축 공기와 100% 산소의 비율을 조절하는 산소 혼합기를 분만실에 비치해야 하는데6) KNN 병원의 구비율도 90%를 넘지 않은 점은 분만실에 산소 혼합기를 시급하게 보급해야 함을 시사한다. 분만병원 중 산소 혼합기 또는 T형 소생기를 구비한 병원에서 소아청소년과 의사 수가 더 많았다. 이는 흡입산소분율을 직접 설정할 수 있는 데다가 안정적인 양압환기를 제공할 수 있는 장비의 임상적 효용을 소아청소년과 의사가 가장 잘 알고 있다는 점에 기인한 것으로 생각한다.

2020년 11월, 보건복지부가 대한소아청소년과학회의 의견을 수렴하여 마련한 ‘신생아실 업무를 위한 일반 신생아 초기 관리 지침’에 따르면, 신생아실 종사자를 대상으로 연 1회 이상의 소생술 교육을 시행하거나 외부 교육을 받도록 지원해야 한다12). 분만병원의 의료인은 정기 교육 기회가 부족했고, 횟수도 지침에 못 미침을 알 수 있다.

통계청과 한국보건사회연구원 발표에 따르면, 한국 신생아의 99% 이상이 의료기관에서 태어난다13,14). 본 연구에서 알 수 있듯이, 분만실 내 소생술을 주도하는 직종은 의사이고, 진료과목별로 소아청소년과를 필두로 마취통증의학과와 및 산과 의사를 포함한다. 따라서, 산모-신생아 진료에 참여하는 의료인은 정기적으로 소생술 교육을 이수하도록 유도해야 한다. 그러나 일부 분만병원에서는 간호사가 소생술을 주도하고 심화한 소생술 단계에서는 팀 단위 숙련도가 중요하므로, 소생술에 참여하는 다양한 직종의 의료인을 대상으로 소생술 교육 기회를 부여하고 자체 교육을 통해 숙련도를 향상하는 체계를 구축하도록 유도해야 한다.

Babbie15)는 사회 조사 설문에서 50% 이상의 응답률이 적절하다고 했는데, 본 연구에서는 분만병원의 비교적 낮은 응답률로 인해 전국 현황을 명확히 파악하기 어려웠다. 그러나 제주도를 제외한 전국 15개 시도에 걸쳐 조사가 이뤄졌고, 사전에 준비된 설문지를 이용하여 단계별로 우편, 전자우편, 훈련받은 조사원의 전화 통화를 이용했다. 이를 통해, 한국 신생아 소생술 지침을 마련하는 데 필요한 기초 자료를 수집했다는 의의가 있다고 생각한다.

본 저자는 대한분만병원협회와 대한신생아학회의 도움을 받아 한국 내 164개 병원을 대상으로 분만실 내 소생술 현황을 조사한 결과, 소생술에 대한 준비와 인식이 미흡하고 이 현상은 분만병원에서 더 현저하다는 점을 알 수 있었다. 향후 한국 실정에 맞는 소생술 지침을 마련하는 데에, 본 연구를 기초 자료로 활용할 수 있다고 생각한다.
