소아 익사의 예후 예측

Abstract

Purpose

Despite the well-known mortality of pediatric drowning, there is a paucity of evidence on the implications of an initial evaluation on the relevant outcome of drowning. This study aimed to investigate the association of initial clinical findings with outcome of children undergoing drowning.

Methods

This retrospective study was conducted using the medical records of 56 children undergoing drowning who visited 3 Korean academic hospitals from January 2000 through May 2020. We analyzed information regarding the prehospital resuscitation, drowning time, a 4-tiered chest radiographic grade, and the baseline characteristics. The grade was defined based on the findings of initial chest radiographs. The poor outcomes were defined as the need for intensive care unit care or death aftercare. We analyzed the association of the prehospital resuscitation, submersion time, and the radiographic grade with the poor outcomes using binary logistic regression.

Results

Among the 56 children, 31 (55.4%) were aged 1-4 years. Prehospital resuscitation and 1-5 minutes of submersion time were noted in the 25 (44.6%) and 30 children (53.6%), respectively. The chest radiographic grades 1 through 4 accounted for 17 (30.4%), 20 (35.7%), 12 (21.4%), and 3 children (5.4%), respectively. Poor outcomes occurred in 17 children (30.4%), including 3 deaths (5.4%). The association with the poor outcomes was noted in the submersion time of longer than 5 minutes (adjusted odds ratio, 21.49; 95% confidence interval, 1.11-415.73; compared with < 1 minute) while not in the submersion time and chest radiographic grade.

Conclusion

This study confirms that submersion time is an outcome predictor of drowning.

서론

익수•익사(drowning)는 액체에 빠짐으로써 호흡곤란 또는 심장정지를 유발하는 환경손상이다[1,2]. 이 손상은 목격자 없이 발생하여 응급실 방문 시 보호자가 진술하는 침수시간이 모호한 경우가 흔하므로, 이를 근거로 예후를 예측하기 어렵다. 소아 익수와 관련하여, 초기 혈당, 글래스고혼수척도, 체온, 동맥혈가스분석과 예후의 연관에 대한 연구는 있었지만[3-6], Graf 등[5]은 방문 직후 혼수상태, 동공반사, 혈당을 이용한 예측모형의 특이도는 100%이지만 민감도는 65%라며 위음성 문제를 지적했다.
한국 국가통계포털에 따르면, 2000-2018년에 0-19세 사인(死因) 중 익사는 3.5% (2,713명)였다[7]. 최근 응급의료체계 발전과 익사 예방 교육으로 익수•익사로 인한 입원이 줄고 있으나, 한국 소아 익수•익사의 이환율, 사망률, 의료기관 이용에 관한 최근 연구는 부족하다[8]. 게다가, 단순흉부방사선사진 소견 또는 병원전 심폐소생술 시행이 예후에 미치는 영향에 대한 연구도 부족하다. 이에, 본 저자는 익수•익사 소아의 응급실 방문 시 단순흉부방사선사진을 포함한 초기 임상증상과 예후의 연관에 대한 연구를 시행했다.

대상과 방법

2000년 1월-2020년 5월에 한국 수도권 3개 통합수련병원의 응급실 또는 외래를 방문하여, 소아청소년과 의사의 진료를 받은 19세 이하 환자를 대상으로 했다. 상기 3개 병원의 기관생명윤리위원회의 심사를 받았고, 후향적 의무기록 연구로 동의서는 면제됐다(IRB no. 2021AS0164).
연구대상자의 의무기록에서 나이(< 1, 1-4, 5-9, 10-14, 15-19세), 계절(봄[3-5월], 여름[6-8월], 가을[9-11월], 겨울[12, 1-2월]), 물의 종류(바닷물 및 민물), 병원전 심폐소생술 여부, 침수시간(보호자 진술, < 1분, 1-5분, > 5분), 의식수준(각성, 기면, 혼미, 혼수)을 조사했다.
응급실 방문 이후 경과 및 호흡 보조 등 중환자실 처치 필요성을 기준으로, “불량한 예후”를 중환자실 입원 또는 치료 중 사망으로 정의했다. 기관내삽관 없이 비침습 산소요법을 시행받은 환자는 불량한 예후가 발생하지 않은 것으로 간주했다. 활력징후와 혈액검사 결과를 조사하여[9] 불량한 예후 발생 여부에 따른 차이를 분석했다.
흉부영상등급은 소아영상의학 전문의 한 명이 4등급으로 구분했다. 1등급은 정상, 2등급은 폐 사이질 음영 증가, 3등급은 간유리음영을 동반한 사이질 음영 증가, 4등급은 다수의 경화(consolidation) 또는 결절성 침윤을 의미하고, 등급이 올라갈수록 심각한 이상을 시사한다(Fig. 1)[10-12].
통계 분석에서 연속형 변수는 Shapiro-Wilk test에서 P가 0.05 이상이면 t-test를, 0.05 미만이면 Mann-Whitney U test로 각각 분석했으며, 범주형 변수는 카이제곱검정 또는 Fisher의 정확성 검정을 이용했다. 병원전 심폐소생술, 침수시간, 흉부영상등급과 불량한 예후(종속변수)의 연관을 나이 및 성별을 보정하여 로지스틱 회귀분석으로 분석했다. 통계 분석에 IBM SPSS ver. 25 (IBM Corp., Armonk, NY)를 사용했다.

결과

1. 환자 분포

연구대상자 56명 나이의 중앙값은 3.5세(사분위수 범위, 2.2-5.4)였고, 1-4세가 55.4%였다. 남자가 62.5%였고, 계절별로 여름이 가장 흔했다. 44.6%에서 병원전 심폐소생술을 시행했고, 침수시간 중 1-5분이 53.6%였다(Table 1). 불량한 예후가 발생한 환자는 17명(30.4%)으로, 모두 중환자실에 입원했고 이중 3명(5.4%)은 사망했다. 불량한 예후가 발생하지 않은 환자는 39명(69.6%)이었는데, 이중 23명(41.1%)은 응급처치를 받지 않았고 16명(28.6%)은 비침습적 산소요법을 받았다. 흉부영상등급은 1단계 32.7% (17명), 2단계 38.5% (20명), 3단계 23.1% (12명), 4단계 5.8% (3명)였다. 4명(7.1%)은 단순흉부방사선사진을 시행하지 않아 분류하지 못했다.
불량한 예후가 발생한 환자는 그렇지 않은 환자에 비해 혈당, 크레아티닌, 알라닌아미노기전달효소 농도가 높았고, 정맥혈가스분석에서 낮은 pH와 더 큰 염기결핍을 보였다(Table 2). 활력징후는 불량한 예후가 발생한 환자에서 산소포화도의 중앙값이 낮은 것 외에는 두 군 간 차이가 없었다(Table 3).

2. 병원전 심폐소생술, 침수시간, 흉부영상등급과 불량한 예후의 연관

흉부영상 중증도 및 병원전 심폐소생술 여부는 예후와 유의한 연관이 없었고, 나이 및 성별 보정 후에도 그 결과는 달라지지 않았다(Table 4). 침수시간은 나이 및 성별을 보정하여 분석한 결과, 5분 초과인 경우가 유일하게 불량한 예후와 독립적으로 연관됐다(보정교차비, 21.49; 95% 신뢰구간, 1.11-415.73, 1분 미만에 비해).

고찰

본 연구는 응급실을 방문한 익수•익사 환자의 예후를 분석함으로써, 침수시간이 사고를 목격하지 못한 보호자의 진술에 따른 변수임에도 예후와 유의한 연관을 보인다는 점을 제시한다. 이 결과는 Suominen 등[13]이 침수시간을 익사의 유일한 예측인자로 보고한 것과 Jeong 등[4]이 28명의 16세 이하 익수•익사에서 침수시간과 예후의 유의한 연관을 보고한 것에 일치한다. 이와 상반되게 Siebke 등[14]이 침수 40분 후에 정상 회복된 증례를 보고했지만, 이는 얼음물에서 발생한 사고이므로 일반화하기 어렵다.
연구 설계 당시에 예측인자로 추정했던 초기 흉부영상등급은 예후와 연관되지 않았음을 확인했다. 본 연구와 달리, Jung 등[6]은 폐부종 환자 17명 중 7명이 사망했음을, Baek[15]은 입원 환자 5명 중 4명이 폐부종으로 집중치료가 필요했음을 각각 보고했다. 본 연구에서 흉부영상등급이 예후와 연관을 보이지 않은 원인은 다음과 같다고 생각한다. 첫째, 사고 후 단순흉부방사선사진을 시행한 시점이 다양했다. 둘째, 침수 초기에 호흡정지, 자발 무호흡, 공황이 일어나며 익사 환자의 약 10%-15%가 흡인 없이 사고 직후 후두경련에 의한 무호흡으로 건성 익수를 일으키므로, 단순흉부방사선사진이 환자 상태를 반영하지 못할 수 있다. 셋째, 단순흉부방사선사진 악화는 임상적 악화보다 느리게 발생한다[15-18]. van Berkel 등[19]은 성인을 포함한 익수•익사 환자의 초기 단순흉부방사선사진 및 동맥혈가스분석 결과가 정상이라도 추후 중증 폐질환을 일으킬 수 있음을 보고하며, 무증상 익수•익사 환자의 입원 결정이 어렵다고 했다. Frates[20]가 소아 소생술 필요성에 대하여 보고한 후, Yang 등[21]은 성인을 포함한 환자에서 적극적 소생술이 좋은 예후와 연관됐다고 보고했다. Kyriacou 등[22]도 익사 소아에서 병원전 심폐소생술을 시행한 환자가 시행하지 않은 환자보다 신경학적 예후가 4배 이상 좋았음을 보고했다. 본 연구에서는 병원전 심폐소생술이 예후와 연관을 보이지 않았다.
익수•익사 환자 예후인자 연구는 지속되어 왔으며, 그 중 Orlowski[23]는 다각적 예후인자로 임상점수체계를 만들어 신경학적 회복 가능성을 예측하려 시도했다. 본 연구에서는 다각적 예후인자 분석을 한 Woo와 Park[24]의 연구에서 착안하여, 폐의 생리학적 변화가 바닷물 및 민물에 따라 다를 것으로 가정하고, 흉부영상등급이 바닷물 여부에 따라 연관을 보이는지 분석했으나, 유의하지 않았다(Appendix 1, https://doi.org/10.22470/pemj.2021.00304). 또한, 바닷물 여부와 불량한 예후의 연관에 대해서는 바닷물에 빠진 환자가 5명에 불과하여 통계적으로 적합한 모형을 얻지 못했다. 최근 바닷물 및 민물(실내 수영장)에 빠진 익수•익사 환자 연구에서 바닷물 여부에 따른 사망 빈도의 차이는 없었다[25].
본 연구의 강점은 21년간 3개 병원 자료를 토대로, 의무기록 및 단순흉부방사선사진을 분석했다는 점이다. 그러나, 연구대상자 수가 비교적 적다는 제한점을 가지고 있으며, 앞으로 한국 소아 익수•익사 환자에 대한 다기관 전향적 연구 및 감시체계가 필요하다.
결론적으로, 본 연구는 익수•익사에서 침수시간이 예후와 연관됨을 확인했다. 따라서, 개인적 차원에서 부모가 익수•익사 발생을 일찍 인지하고, 정책적 차원에서 익수•익사가 흔한 여름에 예방 교육을 강화하면, 예후 향상을 기대할 수 있다.
